3rd Annual L&C National Girls and Women in Sports Day Clinic
This year's event on Sunday, February 4th will feature rotating sports clinics in tennis, soccer, softball, volleyball, track and field and basketball instructed by Lewis and Clark coaches and their players.

Check-in begins at 10:30am in Pamplin Gym and the clinics will start promptly at 11:00am.

Please bring a water bottle and a snack. Girls should dress accordingly to participate in both indoor and outdoor activities (rain or shine). Tennis shoes are required and participants will NOT need cleats or other gear (bats, balls, rackets, etc.) for any of the activities.

Lewis & Clark College: Release of Liability and Indemnity
The undersigned is the parent or legal guardian of ___________________________ (“Minor”), a minor participant in the following program _LEWIS & CLARK NATIONAL GIRLS AND WOMEN IN SPORT DAY CLINIC_ (“Program”).

As the parent or guardian of a minor participant, I understand that Lewis & Clark College (“College”) is providing the venue for the Program, but the Program is being operated by: _L&C ATHLETICS DEPARTMENT_( “Licensee”). Parents and Guardians are frequently worried about minor children participating in programs lead by adults, and the potential for abuse or sexual misconduct. College has required Licensee to certify that all people working for Licensee in the Program as coaches, teachers, and staff members, whether paid or volunteers, have passed criminal background checks and have received training on preventing sexual molestation. However, College does not perform the background checks or supervise the training, and College is not responsible for the conduct of the people working for the Licensee or Program. College’s insurance company requires the parents or legal guardians of minor participants sign this form. Therefor, as a condition of allowing Minor to participate in the Program, I agree to the fullest extent allowed by law for myself and on behalf of the Minor participant, as follows:

1. I release College and its trustees, employees, agents, directors, and officers, and all other persons associated with College including volunteers and independent contractors (the “Released Parties”) from any and all claims which I, the minor participant, or our respective estates or families may have for any loss (including personal injury, sexual molestation or misconduct, property damage, death, or otherwise) arising in whole or in part or in any way related to the Minor’s participation in the Program at the College.

2. I further agree to indemnify (that is, defend and protect by payment or reimbursement including attorneys’ fees and costs) each Released Party from any claim or loss brought by or on behalf of the Minor participant, a co-participant, a rescuer, or any other person including a member of my (or the Minor’s) family, who may assert a loss associated with the Minor’s enrollment or participation in the Program.

IT IS THE INTENTION OF THE UNDERSIGNED BY THIS RELEASE TO FULLY EXEMPT, RELIEVE AND INDEMNIFY THE COLLEGE, ITS PROGRAMs AND ITS TRUSTEES, OFFICERS, AGENTS, SERVANTS, EMPLOYEES, INSTRUCTORS, COACHES, ASSISTANTS, AND VOLUNTEERS FROM AND AGAINST LIABILITY FOR ANY HARM, INCLUDING WITHOUT LIMITATION PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE ARISING FROM THE MINOR’S PARTICIPATION IN ANY PROGRAM AT COLLEGE FACILITIES.

The undersigned has read this release, understands it legal significance, and agrees that this release shall be construed in accordance with, and governed by, Oregon law.
