You have my consent to photograph and video my child(ren). This is including, but not limited to Sunday School, Vacation Bible School, servant events, and party activities. I understand that these photographs/videos may be used in the church building, and publications such as, but not limited to, newspapers, as well as posted on the Good Shepherd Lutheran Church website and websites such as, but not limited to, Facebook.com, and Animoto.com. I understand that the names of my children will not be used in any web site or advertising, I also understand that I will not receive compensation for the use of these photographs/videos and that I will not review or approve the finished product. If at any time I wish to deny Good Shepherd Lutheran Church the consent to photograph or video my child{ren) I understand it is my right to do so by completing a new “Photograph and Video Consent” form.
