Request Assistance Inputting CHIRP Data

IIC’s Data Team is available to help input immunization records into CHIRP. This service is available for organizations in Indiana. To request assistance, simply fill out the form below. After submitting your request, someone will contact you within five business days.

** The Data Team will assist in entering immunization records into CHIRP. During their time at your organization, they will train staff members on best practices for entering future records and running reports. The Data Team will be available to answer any questions, address concerns, and provide resources and helpful information to staff to have on file for future reference.
