Employee Recognition Program 2024
We want to recognize and reward MPS staff members who go above and beyond in their work to make a positive impact on our students and school community. This form is for parents, guardians and community members who want to nominate a staff member for our recognition program. Please use this form to let us know who you would like to see recognized. You must include a phone number (see last question) so we can contact you if we have questions etc. Not everyone nominated is selected (we pick a few at random each week) and we don't identify the person who made the nomination when we share stories about our winners.
