USD #367 Facilities Usage Agreement
Please fill out the below form to submit your request for facility usage. Requests for facility rental must be received no later than 10 days prior to the event in question. Once the form is submitted, it will go to the building administrator/principal of the space requested for review and approval. A $50 deposit is due at the time of request, and payment can be made at the Central Office.  For questions about the facilities, please contact the building principals.
