2020 MAD Camp Registration
Hello, bes!

Thank you for your interest in joining our 2020 MAD Camp.

Please accomplish this form and be reminded of the following:

1. This registration form is just your first step in joining a MAD Camp. Please wait for the confirmation link that we will send via email.

2. Since it's a volunteer program, the volunteers will take care of their own transportation expenses going to and from the camp area.

3. A non-refundable deposit of Php 1,000 must be made upon confirmation and the rest must be paid a month before the scheduled camp.

4. As a group, we will help each other in raising funds for the supplies we will be donating to the children.

5. We will be staying either in a school or in the community, thus we expect limited comfort in the accommodation.

6. Please feel free to invite your friends to volunteer as well. However, do note that we only have a maximum of 25 volunteer slots per camp.

7. Expect our Dream Team to coordinate with you three months before the date of your chosen camp.
