2019 WI Show Member Live Animal & Vendor Table Form 27th July 2019
FMBA Members wanting to vend products, services, or animals at a Fancy Mouse Breeders' Association (FMBA) event must submit this form prior to the show date in order to reserve a vending space at the event. Persons wanting to bring any live animal into the venue area must also submit this form prior to the show date, including animals belonging to member guests. No parking lot or on site exchanges, without going through Health Check. See Show Book for service animal information.
Live animals not being entered in the show require pre-approval for entering the venue area, or vending at the event. Approval does not imply any liability. No parking lot or on site exchanges, without going through Health Check. *
Payment for membership and table space is due prior to the event, via PayPal invoice. Other payment options may be available upon request. Each $30 vending space is one table and two chairs. Tables come with $30 worth of entries. Additional chairs may be available on a first-come first-serve basis, with exhibitors, and requested reservations given priority. Tables may be arranged and assigned the night prior to the event, and vendors may have access to the event space one hour prior to admission of non-vendors, if requested.
FMBA members exhibiting mice, will have priority over FMBA members vending only, in the event that the number of reservations exceeds the number of tables expected to be available. In such cases of no available table, payments made for table space will be refunded. Should the event be cancelled by the FMBA, the FMBA will refund any payments for table spaces that had been made at the time of the cancellation. No other refunds will be made. *
There will be no refunds for those failing health checks, or missed attendance. Any applicable federal, state, or local fees or taxes are the responsibility of the member, as is liability for losses, illness, injury, or damages at the event.

This information is required for health check load, and to protect our exhibitors, and venue agreements. Our health checks are a few volunteers. Too many non-entry live animals can adversely affect the show, and potential willingness of repeat volunteers!
So that the FMBA may be able to provide adequate accommodations and health check, will you be bringing a service animal, as defined by the ADA and the host state's laws?

Predators, especially of mice can be difficult for a health check volunteer safe handling, there may not be a volunteer familiar enough with the animal to adequately health check them, and sometimes are not accepted in the venue at all. It is best if you can make other arrangements for these type of animals off site.
