I grant permission for the management of TLA to obtain medical care for my child as follows:

1. Attempt to reach a parent or guardian, or the person(s) listed as the Emergency Contacts on this Agreement.

2. If TLA is unable to reach any of the above persons, TLA is authorized to accompany and transport my child by TLA vehicle, approved staff member vehicle, or ambulance to the Medical Center Emergency Room for medical care. 911 and/or an ambulance may be called if my child's medical condition warrants it.

3. I give my permission for TLA to give written consent for medical medical care for my child. This includes, but may not be limited to: examination by a physician, laboratory test, x-rays, or other procedures ordered by a physician.

4. Expenses incurred for the above medical care and ambulance transportation is the responsibility of the parents or guardians.

I GIVE CONSENT FOR THE FACILITY TO SECURE ANY AND ALL NECESSARY EMERGENCY MEDICAL CARE FOR MY CHILD.

In the event I cannot be reached to make arrangements for medical care, I request that TLA contact my child's physician (if indicated above).

WAIVER OF LIABILITY: In consideration for my attendance and participation in the martial arts training offered by TALON Martial Arts and American Masters Martial Arts (AMMA), I, the student/parent, acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. I further relieve the school, its management; assigned staff and fellow students from any liability resulting from loss; whether personal belongings or bodily injury. I also hereby state, that myself or my child is physically fit to take the prescribed course of instruction and do so of my own free will.

STATE LICENCING: As a Single Skill Program (Martial Arts), Talon Leadership Academy (Talon Martial Arts) is exempt from licensing by the State of Texas. It is the Talon policy to conduct background checks with the Texas DPS and train our staff in CPR and First Aid.

FEES: Please note that there are 5 situations in which we may assess a $10 penalty. 1) Failure to Notify - this is when your child does not attend school or you pick them up early and do not let us know they do not need to be picked up by TLA after school. We do not leave the school until we know where you child is, causing us to be late for other pickups. 2) Class Interruption Fee - This is when you have to pick up your child and they are in the middle of class and you did not notify us of the same. If you are able to wait until class is over, please do so. 3) Late Payment Fee - Tuition Payments are considered late if they are not paid in full within 3 days of the due date. On the 4th Day your child will be removed from our pickup list and the school will be notified that they no longer attend TLA. 4) Returned Item Fee - If your check, debit card or credit card is charged back for any reason this fee will be applied. 5) Late Pickup Fee - We request that you pick up your child by 6:00 pm. If your child is not picked up by 6:30 pm a Late Pickup Fee will be charged for the first 15 minutes, the an additional charge for each additional 15 minute window.
