Booster Club Funding Request Form
This Google form is used for the purpose of requesting funding from the SHS Booster Club. It must be completed and submitted to the Booster Club no later than the 4th Wednesday of the month.

All funding request approvals will be made based on information from the completed application form, a brief (10-15 minute) question and answer session at an Executive Board meeting as well as funds availability. There is a $1,500 limit per club/organization per school calendar year. Any request over $2,000 or items not considered essential may be considered “wish list” requests by the Booster Club Executive Board.

Has your organization attended a Booster Club meeting once per semester? Attendance may be by an advisor, coach, or representative. Meetings are held the second Monday of the month in the LGI room.
