This form is to apply for Two Teachers Tutoring's Sunray Scholarship for full class ACT tuition.

WHAT IS CLASS ACT?
a five-week Sunday afternoon ACT prep course from 2:00pm-6:00pm in LaGrange at Allegiance Staffing
just prior to an ACT test administration

WHAT ARE MINIMUM REQUIREMENTS FOR CONSIDERATION?
In order to be eligible for the scholarship, applicants must meet the following minimum requirements:

1. 3.4 unweighted GPA
2. eligible for free or reduced lunch
3. one teacher recommendation letter (found at www.twoteachers.org) scanned and emailed to twoteachers36@gmail.com
4. this Google Form application
