Mona Khan Company - Camp Registration
We believe that summer is a special time for children…a time to engage in activities that help them grow, make new friends and have lots of feel-good moments!

CAMP POLICIES:

1. Immunizations are required, and all children must be current on vaccinations in accordance with California state law and local public school requirements
2. Child’s parent or guardian shall provide all medical and emergency information before the child begins camp
3. No medication will be given at camp without written notice from a parent/ guardian and doctor's form
4. All medical forms must be signed by your child’s physician, and must be on file before the start of camp (if child is in multiple camps, one form per camp week is required)
5. Children who are ill or if they have a temperature of 100 degrees Fahrenheit and above / vomiting should not be sent to camp
6. If a child is ill or has a temperature during the day or is vomiting, the parent/guardian will be notified. It is their responsibility to have the child picked up from camp as soon as possible. Children should not return for 24 hours of being symptom free
7. All contagious illnesses a child contracts must be reported to the Camp Management via email
8. If someone other than those listed on the child’s Emergency Information / People Authorized to Pick Up details will be picking up the child, parents or guardians must notify Camp Management via email and in person
9. Every child must bring a water bottle and yoga mat to camp (both items should be labeled with the child’s name)
10. Parents or guardians shall notify the Camp Management via email if a child will be absent on a day they are enrolled in camp
11. Parents or guardians will help and work with the camp staff to handle any behavior problems or other concerns by being available and open to discussions and conferences in order to best service their child
12. Camp timings are 9am-3pm, five days a week. Early drop off from 8am onwards and extended care from 3pm-6pm are offered for an additional fee
13. Canceling Enrollment: To receive a refund, we must receive a written request via email 5 weeks prior to the start of the camp your child is enrolled in. An $80 processing fee will be charged when processing a refund. No refunds will be processed within 5 weeks of camp start date
14. Transfers from one camp to another camp may be accommodated pending availability and other factors, and will be determined solely by Mona Khan Company
15. No part of camp fees (including early drop off, extended care and lunch) will be refunded due to absences
16. All children must be signed in / signed out by a parent or guardian. A photo ID is required during check out at the end of each camp day
17. Children are required to bring lunch from home, or parents / guardians must sign their child up for the lunch program offered by Mona Khan Company
18. Food allergies: We cannot guarantee a food allergen-free environment
19. Lunches brought from home cannot be heated on site
20. The Mona Khan Company reserves the right to cancel a scheduled camp
