IN CONSIDERATION of being permitted to participate in any way in the FACILITY (identified herein as Metasport Indoor Facility) in any activity including but not limited to soccer, speed and agility, private sessions, supplemental classes, or other like activity (herein “Activity”) for myself or personal representatives, assigns, heirs, and next of kin: The Participant and his/her parent(s) or legal guardian(s) certify that Participant has no physical or mental condition that precludes him/her from participating in the Activities and that he/she is not participating against medical advice.
Participant and his/her parent(s) or legal guardian(s) acknowledge, and agree, and represent that I understand the nature of the Activity and that I am qualified, in good health, and in proper physical condition to participate in such activity. I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in the Activity. Fully understand that: (a) soccer, other training, or other like activities, involve risks and dangers of serious bodily injury, including permanent disability, paralysis, and death ("Risks"): (B) these risks and dangers may be caused by my own actions or inaction's, the actions or inaction's of others participating in the activity, the condition in which the activity takes place; or THE NEGLEGENCE OF THE "RELEASEES"NAMED BELOW; (C) there may be other risks and social and economic losses either not known to me or not readily foreseeable at the time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation or that of the minor in the activity. Hereby release, discharge, and covenant not to sue, at all Metasport FC., their Facilities, their respective administrators, directors, agents, officers', members, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owner and lessors of premises on which the activity takes place, (each considered one of the "releases" herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the "releases" or otherwise, including negligent rescue operations and I further agree that if, despite this release and waiver of liability, assumption of risk, and indemnity agreement I, or anyone on my behalf, makes a claim against any of the releases I will indemnify save and hold harmless each of the releases from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim. The Participant and his/her parent(s) or legal guardian(s) understand that Participant’s participation in the activities is voluntary and further understand that they have the opportunity to inspect the host’s equipment and facilities before any participation. The Participant and his/her parent(s) or legal guardian(s) understand that Participant is obligated to follow the equipment rules and the rules of the Activities and that he/she can minimize his/her risk of injury by doing so. I have read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it and have signed it freely and without inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent of the law and agree that if any portion of this agreement is held to be invalid the balance, not withstanding, shall continue in full force and effect.

EN CONSIDERACIÓN de ser permitido(a) participar de cualquier manera en la INSTALACIÓN (identificada en este documento como la Instalación Techada de Metasport) en cualquier actividad, incluyendo pero no limitándose a fútbol, entrenamiento de velocidad y agilidad, sesiones privadas, clases suplementarias, u otras actividades similares (en adelante, “Actividad”), para mí o mis representantes personales, cesionarios, herederos y parientes más cercanos:
El Participante y su(s) padre(s) o tutor(es) legal(es) certifican que el Participante no tiene ninguna condición física o mental que le impida participar en las Actividades y que no está participando en contra del consejo médico.
El Participante y su(s) padre(s) o tutor(es) legal(es) reconocen, aceptan y declaran que entienden la naturaleza de la Actividad y que están calificados, gozan de buena salud, y se encuentran en condiciones físicas adecuadas para participar en dicha actividad. Además, acepto y garantizo que si en cualquier momento considero que las condiciones son inseguras, dejaré inmediatamente de participar en la Actividad.
Entiendo completamente que:
(a) el fútbol, otros entrenamientos u otras actividades similares implican riesgos y peligros de lesiones corporales graves, incluyendo discapacidad permanente, parálisis y muerte ("Riesgos");
(b) estos riesgos y peligros pueden ser causados por mis propias acciones u omisiones, las acciones u omisiones de otros participantes, las condiciones en las que se lleva a cabo la actividad, o la NEGLIGENCIA DE LOS "LIBERADOS" NOMBRADOS A CONTINUACIÓN;
(c) puede haber otros riesgos y pérdidas sociales y económicas que no conozco o que no son fácilmente previsibles en este momento; y acepto plenamente y asumo todos esos riesgos, así como toda la responsabilidad por pérdidas, costos y daños que pueda sufrir como resultado de mi participación o la del menor en la Actividad.
Por la presente libero, eximo y me comprometo a no demandar a Metasport FC, sus instalaciones, sus respectivos administradores, directores, agentes, funcionarios, miembros, voluntarios y empleados, otros participantes, cualquier patrocinador, anunciante, y, si aplica, el propietario o arrendador del lugar donde se lleva a cabo la actividad (cada uno considerado como uno de los "liberados" en este documento) de toda responsabilidad, reclamaciones, demandas, pérdidas o daños en mi contra causados o supuestamente causados total o parcialmente por negligencia de los "liberados" u otros, incluyendo operaciones de rescate negligentes.

Además, acepto que si, a pesar de esta renuncia de responsabilidad, asunción de riesgos y acuerdo de indemnización, yo o cualquier persona en mi nombre presenta una reclamación contra cualquiera de los liberados, indemnizaré, salvaré y mantendré indemne a cada uno de los liberados de cualquier gasto legal, honorarios de abogados, pérdidas, responsabilidades, daños o costos que puedan surgir como resultado de dicha reclamación.
El Participante y su(s) padre(s) o tutor(es) legal(es) entienden que la participación del Participante en las actividades es voluntaria y además comprenden que tienen la oportunidad de inspeccionar el equipo e instalaciones del anfitrión antes de cualquier participación.
El Participante y su(s) padre(s) o tutor(es) legal(es) entienden que el Participante está obligado a seguir las reglas del equipo y las reglas de las Actividades, y que puede minimizar su riesgo de lesión al cumplirlas.
He leído este acuerdo, entiendo completamente sus términos, entiendo que al firmarlo estoy renunciando a derechos sustanciales y lo firmo libremente y sin ningún tipo de incentivo o garantía de ninguna naturaleza, y tengo la intención de que sea una liberación completa e incondicional de toda responsabilidad en la mayor medida permitida por la ley, y acepto que si alguna parte de este acuerdo se considera inválida, el resto continuará plenamente vigente y en efecto.
