1. ALL VISA CLASSES: REQUIRED ORIGINAL DOCUMENTS

International passport (copy of biodata page with your photo on it required)
Two (2) passport photos: Photos must show the applicant’s full face on a white background. The required size is 1.5 x 1.5 inches (5 cm x 5 cm). See our photo guidelines resource page for more information.
Online application form: Fill out the online application Form DS-260. You will need to contact NVC to get your invoice ID number.
Birth certificate: All beneficiaries must submit a valid birth certificate. If born in Nigeria after 1992, it must be a National Population Commission (NPC) birth certificate. If born in Nigeria prior to 1992, you must obtain an Attestation of Birth from the NPC.
Petitioner’s Birth Certificate (for all IR5 parental cases).
Marriage Certificate (If applicable)
Marriage Termination/Divorce Decree (if applicable) (for beneficiaries who have a U.S. citizen spouse or fiancé(e) petitioner, divorce certificates are required for all the petitioner’s and beneficiary’s prior marriages).
Death certificates (if applicable, for prior spouses of beneficiaries and petitioners)
Police Certificate: Please submit a police certificate for ALL countries in which you have resided for more than six months after the age of 16. (If you hold citizenship in another country outside of Nigeria, you will have to provide a police certificate for that country of citizenship, if you resided there for a period of six months or more). Please note that only police certificates issued within the past 24 months are considered valid.
Court and Immigration Records: If you have been convicted of a crime or other offenses, in the United States or elsewhere, obtain a certified court record covering each offence. If you previously resided in the U.S., you are encouraged to bring evidence of your immigration status during your stay, and records of any previous U.S. visa extensions and deportations that would be relevant for review.
Other Supporting Documents: Bring evidence of the ongoing relationship you have with the Petitioner, including but not limited to:

Photographs
Chat logs from social media or other communications applications (DO NOT print them on a Word document. Logs MUST be printed directly from the site).
Phone records showing both phone numbers and the duration of the calls.
Email correspondence (DO NOT print them on a Word document).

2. ALL VISA CLASSES (EXCEPT K1, SEE 5 BELOW): REQUIRED ORIGINALS AND COPIES

Form I-864 Affidavit of Support (AOS): Your Petitioner must complete an AOS. Please visit the USCIS website for detailed information about AOS requirements and forms.
Joint Sponsor: Note that if your Petitioner does not meet the income requirement, you will need a joint sponsor to complete an AOS. IRS Tax Transcript: Your petitioner (and joint sponsor, if necessary) must submit an IRS tax transcript for year 2021. They can request a tax transcript at https://www.irs.gov/individuals/get-transcript.

3. EMPLOYMENT VISAS: REQUIRED ADDITIONAL ORIGINALS AND COPIES

NURSES: If you are migrating based on your qualifications as a nurse:

U.S Nursing License
Letter of Employment: Please bring a recently-issued letter of employment from both your staffing agency AND the hospital you have been assigned to detailing your pay rate and work location address.

ALL OTHERS: If you are migrating based on your skilled profession (not nursing):

Any relevant higher degrees or professional certificates
Evidence of your prior employment and experience in your field
A recently issued job offer letter from the employer who is sponsoring your immigration detailing your anticipated work duties, the address of your work location, and your pay rate.

5. FIANCE CASES: REQUIRED ORIGINALS AND COPIES

IV Processing Fees: Visit any branch of Guaranty Trust Bank PLC in Nigeria to pay the visa processing fee of $265.
Form DS-160: Please submit the online DS-160 visa application by following the instructions at https://ceac.state.gov/GenNIV/Default.aspx. Please note the form is only valid for one year. If you submitted the form one year ago or more, please re-submit.
Form I-134 Affidavit of Support (AOS): Please have your Petitioner complete an AOS. Download Form I-134.
Joint Sponsor: Note that if your Petitioner does not meet the income requirement, you will need a Joint Sponsor to complete an AOS and provide proof of income.
IRS Tax Transcript: Your petitioner (and joint sponsor, if necessary) must submit an IRS tax transcript for year 2021. They can request a tax transcript at https://www.irs.gov/individuals/get-transcript.

5. ADOPTION CASES: ADDITIONAL REQUIRED ORIGINALS AND COPIES

Birth Certificate: All beneficiaries must submit a valid National Population Commission birth certificate issued after the adoption has been finalized.

I-765 form- Application for Employment Authorization: You must fill and present this form.
G-325C Form Biographic Information: You must fill and present this form.
Notice and Declaration on Pre-departure Marriage Form: Unmarried beneficiaries who are 14 years and older must fill this form.
Evidence of relationship between you and your petitioner: Examples include photos, chat records, phone records, money transfer receipts, etc.
Medical CD: Beneficiaries 15 years and older are required to present the CD enclosed in the medical report at the time of interview. It is your responsibility to collect it from the panel physician.
