Are you enrolled in our cheer program and ready to meet your teammates?  Are you interested in joining our cheer program, but not sure what it is all about?  You can attend our free cheer clinic and learn hands on!  We will run through a typical practice day and teach proper technique of motions, stunts and tumbling.  We will also learn fun sideline material.

Waiver:  I the undersigned participant and parent/guardian of the above listed minor (if participant is under the age of 18) acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability or death, and severe social and economic losses which might result not only from their own actions, inaction's or negligence, but action, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used and further, that there may be other unknown risks not reasonably foreseeable at this time, assume all the foregoing risk and accept personal responsibility for the damages following such injury, permanent disability or death, hereby release, discharge, covenants to indemnify and not to sue the  Lake Nona Youth Sports INC, its directors, board members, officers, employees, coaches, managers, agents, sponsors and associated personnel including those of its affiliated organizations, and the owners and lessor’s of premises used to conduct the event, all of which are hereinafter referred to as 'releases', from any and all liability to each of the undersigned, his/her heirs or next of kin for any and all  against any claim by or on behalf of the applicant as a result of the applicant's participation in the Programs and/or being transported to or from the same, which participation, after careful consideration I hereby authorize, and which transportation I hereby authorize. The applicant/participant has received a physical examination by a physician and has been found physically capable of participating in the Programs.  I hereby give my consent to have an athletic trainer, coach and/or doctor of medicine or dentistry or associated personnel to provide the applicant/participant with medical assistance and/or treatment and agree to be financially responsible for the cost of such assistance and/or treatment.  I, also agree to save and hold harmless and indemnify each and all parties herein referred to above as releases from all liability, loss, cost, claim or damage whatsoever, including death or damage to property, which may be imposed upon said releases because of any defect in or lack of such capacity to so act or caused or alleged to be caused in whole or in part by the negligence of the releases.  I understand that this document may not be altered in any manner and that any alternation without the express written consent from the Lake Nona Youth Sports, INC will cause the participant to be removed from the Program. *
For Disciplinary Actions:  Parent and/or athlete may request a meeting with the grievance committee to review and discuss the disciplinary action.  The board will have final authority on penalty.  This action is reserved for suspension situations. For issues or concerns regarding your athlete’s participation with the Lake Nona Youth Sports, INC organization:The board members of Lake Nona Youth Sports INC are dedicated to providing a quality sports program to the youth of Lake Nona.  We understand youth athletics is a highly energized atmosphere that can create difficult situations.  We desire everyone to have an equal voice and opportunity to resolve conflict and frustrations.  The following steps are outlined to help resolve grievances.  Any concerns regarding the individual team should first be addressed to the head coach.  If you are unable to resolve the issue with the head coach then concerns may be addressed to the VP of football, VP of cheer, VP of Lacrosse, VP of Wrestling and VP of Basketball. We ask parents to make every effort to resolve concerns with the head coach first. Any concerns regarding the conference can be addressed to any board member. If you witness any offensive language or acts by another person at practice, a game, or any league function please advise a board member immediately. *
Disciplinary and Grievance Policy:  Any behavior that violates the athlete code of conduct will be considered an offense.  The following guidelines will be utilized with respect to the severity of the infraction.  Any disciplinary action will be at the discretion of the coach and board. First offense-verbal warning to athlete and parent notification. Second offense- written warning; loss of game time (minimum of 1 game and will be at the board's discretion). Third offense- suspension from league for the remainder of the season pending completion of the grievance process. Any athlete suspended from school will automatically be suspended from Lake Nona Youth Sports INC pending completion of the grievance process. *
Promotional Consent:  I hereby grant to Lake Nona Youth Sports INC the absolute right & permission to make, reproduce, broadcast, or otherwise use participant’s name & likeness, any photograph, films, videos, recordings, or other depictions or images in whatever form or media in connection with participation throughout the universe in perpetuity & in any & all advertising & promotion materials, in any manner or media whatsoever for purposes of art, advertising, editorial, trade or promotion or any other purpose whatsoever. *
