Filling out this document is essential for our staff to review your venture for potential participation in our accelerator program. While many of the questions are optional, please tell us as much as possible about your venture or venture idea. Depending on our evaluation of your questionnaire responses, we will contact you to schedule an intake interview with us as the next step in the process.

Again, please BE REALISTIC and note that this is a PROJECTION, not an actual expense figure.
Please tell us how long you've been working on the venture, and approximately how many hours per week have you been working on it?
Please tell us in 3 sentences or less how your venture is sustainably differentiated from its competition (i.e., how you are different from the competition in a way that will be hard for your competitors to imitate).
Please tell us what major resources you have (physical facilities, equipment, vehicles, computers/phones/tablets, patents/trademarks/copyrights, etc.). If you don't have any major resources, please list the resources you'd LIKE to have and note that they are targets rather than actual resources.
Please tell us the amount of cash you can access from your own personal finances or from friends & family to use on this venture.
What goal would you like to achieve upon completion of your participation in our accelerator program (i.e., what will the launch date for your business be, what level of monthly revenues do you want to reach, what new product/service do you want to launch, what new customers do you want to acquire, what new employees do you want to hire, what new business partners do you want to acquire, what level of investment do you want to attract, etc.)?
