I understand that there is a risk of injury associated with participation in physical activities and the use of interactive equipment such as the equipment my child may use at Bulldog for Kids.
I agree to waive any and all claims of liability, release and hold Bulldog For Kids and its associated companies harmless in the event such injury occurs to my child, meaning I will not sue Bulldog Fitness or try to hold Bulldog For Kids legally responsible.
In the event of an accident or injury when parent, legal guardian, or emergency contacts are not available, I give my permission to Bulldog For Kids to obtain whatever medical attention it deems necessary and assume all costs associated with such treatment.
Bulldog For Kids is not responsible for any lost or stolen items left at the Bulldog For Kids.
I grant Bulldog For Kids my permission to use the photographs taken at events, programs, parties and any other program offering for any legal use, including but not limited to social media, publicity, copyright purposes, illustration, advertising, and web content. Photos will not be sold to other companies.
Furthermore, I understand that no royalty fee or other compensation shall become payable to me by reason of such use.
I have assessed my child(ren) for Covid-19 symptoms prior to our arrival and my child(ren) remains symptom free. My child(ren) has not been in direct contact with an individual who tested positive for Covid-19 within 7 days.
