Post-Match Code Violations
This form shall be used to inform the ITA Officiating Department of any code violations that occur after a match is over. Essentially, this would be considered a carryover penalty, but because the match is over, there is nowhere for it to be applied. The ITA would like to be aware of any issues that arise once a match is over to determine if any additional action needs to be taken. The actions do not necessarily need to be at the level of an offense that would qualify for an immediate default; any behavior worthy of a code violation should be reported using this form.
