TeleHEALTH Consultation Request 

Please complete the following form to facilitate the arrangement of your consultation appointment. Our staff will contact you to confirm the appointment and provide any additional details as necessary.

**TeleHEALTH is only available to pets who have been seen physically at SingVet. 

Preparation Requirements:

  1. Ensure you have a laptop, tablet, or smartphone equipped with a camera and microphone capable of supporting the video conferencing platform.
  2. We offer communication via Zoom, or WhatsApp calls. Please download the relevant app on your device before the appointment. A link for the video conference will be emailed to you closer to the appointment time if applicable.
  3. Accurately weigh your pet to one decimal point, as this is crucial for precise medication dosing. For pets over 3 kg, use a digital bathroom scale: weigh both yourself and your pet together, then subtract your own weight taken immediately after. For pets under 3 kg, a kitchen scale will provide more accurate results. If medications are prescribed, an estimate will be emailed to you. Upon confirmation and payment, arrangements for self-pickup at the clinic can be made.

For any recommended test samples (e.g., fecal or urine), please drop them off at the clinic. A veterinarian will follow up with you via phone regarding test results and further treatment recommendations.

DISCLAIMER Please be aware that video consultations have limitations. The inability to conduct a physical examination may affect the accuracy of diagnosis and assessment of your pet’s health. Additionally, the quality of the video can impact the effectiveness of the consultation. Telemedicine serves as a tool to determine whether immediate or future in-clinic evaluation is needed, or if medication can be prescribed.

If a physical visit is deemed necessary after the telemedicine consultation, the fee paid for the telemedicine session can be credited towards the in-clinic consultation fee if used within 7 days.

All telemedicine consultations will be recorded in your pet’s clinical history, as with any physical visits.
