I, on behalf of myself, as a condition of my participation hereby waive any and all claims I may have now or in the future against my instructor and Story Yoga Inc. and The Turning Point Center of Chittenden County in connection with or arising out of my participation in classes online, recorded, or in person. I hereby acknowledge the following notices and grant to the instructor and Story Yoga Inc. and the Turning Point Center of Chittenden County the following release from liability: I acknowledge and fully understand that I will be engaging in physical activities. I acknowledge and have been advised that it is my responsibility to consult with my or my physician or health care provider with respect to any past or present injury, illness, health problem or any other condition or medication that may affect participation. I assume the foregoing risks and accept full personal responsibility for any personal injuries sustained which might incur as a result or participating in this program and discharge and hold harmless Story Yoga Inc. and the Turning Point of Chittenden County from any claim, cause of action or liability for damages arising from any personal injury property caused by my participation in online, recorded, or in person classes. I agree to assume all full responsibility for all risks, conditions, injuries or damages, known or unknown, which I might incur or aggravate as a result of entering or being on the premises or participating in the classes offered online, recorded, or in person. I represent that I am in sufficient physical health and have no medical conditions that would prevent participation in the classes that I have chosen to take at Story Yoga Inc. or the Turning Point Center of Chittenden County.-- The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Story Yoga Inc. and the Turning Point Center of Chittenden County has put in place preventative measures to reduce the spread of COVID-19 for their in person classes; however, we cannot guarantee that you will not become infected with COVID-19. Further, attending our in person classes could increase your risk of contracting COVID-19. By attending these classes, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk you may be exposed to or infected by COVID-19 by attending in person classes offered by Story Yoga Inc. and the Turning Point Center of Chittenden County and that such exposure or infection may result in personal injury, illness, permanent disability, and death. You understand that the risk of becoming exposed to or infected by COVID-19 when attending Story Yoga Inc.'s or the Turning Point Center of Chittenden County's in person classes may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Story Yoga Inc.’s and The Turning Point Center of Chittenden County's employees, teachers, and program participants and their families. You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that you may experience or incur in connection with your attendance and participation at these classes. You hereby release, covenant not to sue, discharge, and hold harmless Story Yoga Inc, The Turning Point Center of Chittenden County's, their employees, agents, and representatives, of and from all claims, including all liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto. You understand and agree that this release includes any claims based on the actions, omissions, or negligence of Story Yoga Inc and the Turning Point Center of Chittenden County its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in in person classes and programs. I have carefully read, understand, and agree to the contents of the release and waiver of liability. I voluntarily and of my own free will sign and agree to the terms and conditions stated above. *
