More Info:

This registration form must be completed by an adult (age 18+).
A parent/legal guardian must complete this form for minors (under 18 years of age).
Children age 12 and under must be accompanied by an adult at the Beach Sweeps.
Beach Sweeps participants are required to sign a waiver of liability and photo release prior to or at the time of check-in at the Beach Sweeps.
