I give New Creation Dance Company and its members permission to take pictures/videos of myself/my dependent during New Creation dance activities (including dance classes, workshops, fundraisers, rehearsals, and performances), and also to use those pictures/videos for promotional purposes on their website, social media pages, fliers, or for any other form of advertising in the future.
I understand that dancing has associated risks, including that of physical injury. I therefore agree to assume all such risks, and exempt, release, and indemnify New Creation Dance Company and its members from any liability loss or injury to me/my dependent or my (dependent’s) personal property which may occur due to any cause in relation to participation in New Creation activities (including dance classes, workshops, fundraisers, rehearsals, and performances). *
