Please complete a separate form for EACH quilt you are submitting. Though there is no limit to the number of quilts you can enter, please understand that due to space limitations, you may be asked to prioritize your submissions.

You will receive an email with a copy of this completed form after you submit it.

After you submit your form, please email FVQuiltRegister@gmail.com and attach a photo of your quilt. Be sure to name your photo in such a way that it is not confused with other quilts. You will receive a confirmation for each quilt by email.

The Farmington Valley Quilters Inc. has your permission to photograph your work for publicity purposes, to use clips and pins to display your work. The FVQ reserves the right to reject work felt to be inappropriate, in poor condition or unfinished.

Quilts by non-members (including family members) may need to be eliminated if space is limited.
