• Motivation to become a ham and interests in the hobby • Ham radio activities and accomplishments • Ham radio contacts and operating modes • Interests and hobbies outside of ham radio • Ham radio club memberships • Ham radio club participation (offices held, appointed positions) • Any special considerations that affect your application and that you would like us to take into consideration • Enrollment status detail, if you are not a full-time student • High school academic honors • College academic honors
If you cannot use our on-line application, please contact me directly (firstname.lastname@example.org) and provide an explanation of the issue(s). If necessary, I can provide an alternate form of the application to you.
Ultimately, it is our goal to have all applicants use this on-line form. Hence, it is important for us to understand your issues with the use of the form and any problems that you might have encountered when using it.
Your application form data goes directly into an encrypted, password protected PDF file that is available only to the review committee. Your raw input data is not stored on-line.
If you would like to have a copy of the completed application, please send the request to email@example.com. You will receive an encrypted, password protected PDF copy by return email. The password will be sent in a latter transmission.
