Medfield Afterschool Program Jump Start Registration 2021-2022
Jump Start MAP is for children who are 4 & 5 years old and not yet in Kindergarten.  Children participating in the Memorial School Integrated Preschool, can be picked up or dropped off from MAP on their scheduled days.
MAP must have a minimum number of children enrolled in order for the program to run each day. Please complete one form per child.  Please complete by Friday, May 7, 2021. Thank you!

JUMP START MAP - Mornings - 8:30 a.m.-12:15 p.m.   Please select the days you are interested in registering your child (if attending the Integrated Preschool @ Memorial in the afternoons, MAP staff will walk the children to preschool at 12:15) *

To complete the registration process, please mail MAP a $50.00 registration fee.  This fee is only deposited if you enroll in MAP.  Checks should be payable to MAP and mailed to P.O. Box 18 Medfield, MA 02052. Please write your child's name in the memo section of your check.  Please Type your name below as your signature.  Thank  you!
