CD4 DFL Officer Screening Questionnaire
Screening - Please fill out this form if you would like to be considered by the CD4 Business Conference Nominations Committee for recommendation to any of the below positions at the business conference on May 4th, 2025 at the Ramsey County Library, 2180 Hamline Avenue North in Roseville. Registration will begin at 12:30 PM, and the business conference will be called to order at 1:45 PM. 

The Chair and Vice Chair must be gender balanced. The vacancy on the State Nominations Committee must be filled by a non-male.

Note that screenings will be done via Zoom and a link with a Calendly survey will be sent for you to select a one hour block that works for your schedule. Your screening will be 10 minutes and after you select the date and hour that works for you, the Nominations Committee will confirm that there is capacity for your screening. Screenings in a given hour will be conducted on a first come basis based on the number of available committee members to conduct the screenings.

Further, note that you do not need to be present at the business conference to be recommended by the committee or to be elected at the business conference.
