This form gathers essential information to support a safe and appropriate referral into Raw Material’s programmes. The details you provide will be used only for assessing the referral, planning support, and fulfilling our safeguarding responsibilities.
All information will be stored securely, kept confidential and only shared externally where required for safeguarding or legal purposes. No identifiable information will appear in any reports.
We are unable to accept referrals for participants who:
• Have a history of sexual offences or sexual misconduct.
• Have a criminal record within the past 5 years related to violent or high-risk behaviour towards others.
• Have engaged in behaviours considered high risk towards themselves or others within the preceding 12 months and are likely to require professional mental-health and/or social-care support.
Providing detailed and accurate information enables us to carry out our safeguarding procedures and ensure the best possible support for your client.
Please write out any acronyms in full. This applies to conditions and diagnoses as well as the names of services, teams and organisations - for example, write "Child and Adolescent Mental Health Services (CAMHS)" rather than "CAMHS".
Please give clear dates for anything you describe. Where you refer to a service the participant has engaged with, or to a past incident, admission or activity, say when it happened and over what period rather than "recently" or "in the past".
