The Property
Located in the beautiful Village of Mount Pleasant, Wisconsin.  One outdoor parking space is provided with each apartment.  An Indoor Garage space is optional to rent at $75.00 per month.
Landlord requires a minimum year rental agreement, with a month-to-month rental afterwards, requiring a 60-Day Notice at the end of the initial year and the month-to-month rental term period after that.
Included In The Rental Rate
1.  Hot and cold water.
2.  Garbage removal.
The following the tenant pays separate from rent:
1.  Electricity:  Heat, appliances that run on electricity which are all appliances (landlord provides the actual appliances as existing in the apartment). 
Pets
Landlord allows only fish or birds as pets, or a cat with a monthly pet rental fee paid.  Sorry, no dogs are permitted, except for a SAA or ESA per the below information.
Service or Assistance Animal ("SAA") or Emotional Support Animal ("ESA")
If applicant declares the need for a Service or Assistance or Emotional Support Animal, applicant must provide the following with the application:

A. A signed, dated, written and notarized statement from a licensed doctor or licensed physician which is physically located in the State of Wisconsin, or from another State in which the applicant is originating, the Emanating State, on the doctor’s or physician's official letterhead indicating the need for an SA or ESA.

B. Copy of the practicing physician’s or doctor’s current and valid License issued by the State of Wisconsin (this must come directly from the State of Wisconsin or other relevant State authority), or if meeting the Emanating state requirements, the relevant state, with a certificate or documentation from the relevant State authority that the physician or doctor is in good standing.

Misrepresentation(s) by the applicant to the landlord regarding this policy is subject to both civil and criminal pursuit of prosecution for such misrepresentation(s).  Applicant must declare in advance to landlord that the tenant is in need of a SAA or ESA, unless the need is readily apparent, i.e. a Seeing-Eye (TM) dog is a readily apparent need.   NOTE:  Due to limitations of staffing and allergic reaction concerns, tenant will have to abide by Landlord's conditions of rental regarding Service or Assistance or Emotional Support Animal.  See the Residential Rental Agreement and Nonstandard Rental Provisions for full details.

One Bedroom: Up to 1 adults and 1 minor.  (Only one 1 Bedroom unit available at the property at 1120 Oakes Road Building.)
Three Bedroom: Up to 4 adults, or 2 adults and 4 minors.
Adult: 18 years of age or older. Minor: Under the age of 18.

† Additional rent tied to adult occupancy is based on an economic model to account for additional use of included utilities according to the number of adults in tenancy.

NOTICE:  HAVING MORE PERSONS THAN THE ALLOWED PARAMETERS, OR INDIVIDUALS NOT LISTED ON THE RENTAL AGREEMENT WILL SUBJECT THE TENANT TO ADDITIONAL FEES/DAMAGES/CHARGES AND/OR EVICTION.

Furniture Size Limitation
Due to clearances of entrances, stairwells and hallways, couches (COUCHES ARE STRICTLY limited to 2 seat, or love seat, size) or other furniture are limited in length to 60" (60 inches) for any single piece of furniture or section of furniture that can be disassembled and reassembled in the apartment.  Generally, this limitation would not apply to the measurements of a mattress, since mattresses are long but thin, but such distinction MAY NOT be applied to other pieces of furniture
Credit and Background Check Fee or Report Required
BEFORE Applicant submits any deposit money as noted in the next section Applicant MUST PAY for a $25.00 NONREFUNDABLE Credit Check and $10.00 Background Check Fee (Total of $35.00 per applicant) AND/OR agrees to supply a verified credit report dated within the last 30 days to Landlord to the following email:  daycompany@gmail.com

Renter's Insurance Required
Each tenant is required to carry his or her own renter's insurance, and Day Company must be named as a party to be notified of any changes to the policy, i.e. cancellation, non-payment of premium, claim of loss, etc.  Applicant will be required, upon approval of the application, to submit proof of insurance to Day Company in the form of a Certificate of Insurance to be sent to Day Company via email to:  daycompany@gmail.com or via U.S. Mail to:  Day Company, Post Office Box 170706, Milwaukee, WI 53217-8061.   Note:  Renter's Insurance is usually available for a cost of between approximately $10.00 to $20.00 per month, depending on the coverage, so the applicant is encouraged to take this amount into account when planning monthly expenses of renting.
Income Requirement
Income requirement:  We require a minimum of three times the total rental payments for the year of rental of the apartment as a minimum income.  For example, if the apartment rental rate is $1,395.00 per month, the income required would be $1,395.00 x 12 months x 3 (multiplier) = $50,220.00 yearly income required.  Income sources and assets must be subject to USA jurisdiction, i.e. located in the USA, which precludes foreign sources of income, and precludes embassies, missions or other diplomatic status.   Applicant must provide proof of income by sending scanned copies of any documentation to support your declared income, i.e. pay stubs, educational grant and/or loan statement(s) or certification letter(s), etc. to the following email:  daycompany@gmail.com.  Please include applicant's name in the subject line of your email.

Credit Report and Background Check Conditions
Landlord adheres to the following basic requirements, subject to the review of your application in detail:
1.  No evictions.  If there is a current case pending, the application will be held in abeyance until the case is concluded.
2.  Minimum credit score of 650.  (Applicant may provide a Cosigner that meets a 700 minimum credit score requirement.)
3.  No delinquent housing accounts.
4.  No rental judgements.
5.  No outstanding utility bills.
6.  No bankruptcy filings.
7.  Less than $3,000.00 in past due/in collections for entire household.

A Cosigner is someone who will guarantee the apartment rental payments and the financial obligations of the agreement.  We have credit background requirements where applicant may choose to provide a Cosigner in order to guarantee the rental payments.  Each applicant must meet this requirement, since the application is joint and several; this means that each applicant is fully responsible for the rental payments, so if one tenant doesn't pay, each person is responsible for the full payment until paid in full.  If applicant is unable to meet the credit requirements, applicant must have a Cosigner; a Cosigner must have an income 4x the total rental payments due on the apartment.  For example, if the rental on the apartment is $1,025.00, then Cosigner's income must be $1,025.00 x 12 months = $12,300.00 x 4 (multiplier) = $49,200.00.  Otherwise, applicant can submit a deposit in an amount equal to 2x the total monthly rental on the apartment; for example, for a 3 Bedroom: $1,025.00 x 2 = $2,050.00 deposit required.    Cosigner must be subject to USA jurisdiction, i.e. located in the USA, which precludes foreign embassies, missions or other diplomatic status.

Terms, Conditions and Signature
Applicant understands this is an Apartment Application (“Application”) for an apartment rental and has read all pages, which are the sole Terms and Conditions of application. Landlord may undertake at any time a credit and background investigation of persons listed herein, including any history and character. Information may be exchanged now and in the future between all relevant parties deemed so by Day Company. All information on this form, or attached to it, is true, complete and correct to the best of applicant’s belief and knowledge and that if any information provided is found to be untrue, incomplete or misleading, this will be cause for landlord to reject the application or terminate the tenancy, even after occupancy of the apartment. A 60-day written notice from the 1st of the month is required at the end of the rental term or at any time thereafter. Landlord makes no promises of any aesthetic improvement to the apartment. Applicant must maintain personal property, liability, accident insurance at all times; landlord is not responsible for damage, injury or loss, not due to landlord’s negligence; presumption is landlord is not negligent. All rental payments are due in advance on or before the 1st of the month; Check or Money Order ONLY (NO CASH) payable to: Day Company. Applicant certifies Applicant was shown the apartment indicated above, or any subsequent to which Applicant occupies, furnished for inspection a copy of the Residential Rental Agreement and Nonstandard Rental Provisions, which are also available online at the following website: daycompanymilwaukee.com, and Applicant agreed to the Terms and Conditions before Applicant gave a deposit or submitted an application. Within 7 days of the start of tenancy, tenant may request, in writing, that landlord provide to tenant a list of physical damages or defects, if any, charged to prior tenant’s deposit. Landlord requires 1-10 business (Monday through Friday, excluding Holidays) days to review and act on the application, excluding any government agency approval needed or required, which applicant agrees may extend such time. If applicant’s application is denied, Landlord will return entire deposit amount within 7 business days to 1st Applicant address listed above. If Applicant cancels, withdraws or otherwise fails to complete the application process, the DEPOSIT herewith shall be treated as damages, and to cover landlord’s administrative costs or other labor time involved. If Applicant is approved, but fails to complete the Residential Rental Agreement and/or occupy the apartment, Applicant is liable for rent until re-rented, and the DEPOSIT herewith shall be treated as a RE-RENTING FEE and is a part of mitigation of damages.
