Youth Choir Registration Form
This is the registration form for participation in Seraphic Voices (Youth Choir) and the CCSMAA Music Program. Seraphic Voices is open to youth ages 9–17. At the discretion of the Choirmaster, children younger than age nine may be allowed to join the choir.

In order to reduce paper usage, we're collecting all contact information digitally. Please complete the form and submit before 14 September 2017. Even if the church has your contact information, please include it here. This form is secure and information will remain confidential unless permission to share is received. By completing this form, you are consenting to have your email address added to our CCSMAA Music Program mailing lists.

To the Parent/Guardian: Are you willing to volunteer as a Choir Parent at one rehearsal every six weeks? A Choir Parent observes rehearsal, assists at dinner, and assists in non-musical activities. *
To the Parent/Guardian: Are you willing to occasionally serve as a Choir Parent at the services the choristers sing? Choir Parents observe warm-ups/rehearsals, assist in vestment distribution, and assist in music distribution and collection. *
To the Parent/Guardian: Are you willing to prepare, serve, and clean-up a dinner for the choristers once every six weeks? Meals are to be planned in advance with Matt, the choirmaster. *
To the Parent/Guardian: Are you willing to arrange occasional social gatherings away from the cathedral for the choristers? (Possibly once or twice a year.) *
