Please read carefully before signing

By signing this form, I give permission for my child(ren), to participate in activities organized by Sonrise Christian Reformed Church (“Sonrise”), including transportation to and from the church.
I understand and accept the risks involved, including injury, disability, or death, whether due to negligence or other causes. I agree not to sue Sonrise or its staff, volunteers, or associated persons (“Releasees”) for any injuries, losses, or damages that may occur.

GEMS Media and Promotions

I give permission for the GEMS Club to use videos/pictures to be taken and used for GEMS media and promotions of the club an for her first name only to be used in identifying her. I hereby release the Sonrise Ponoka GEMS Club from all claims and liabilities arising out of the use of photos and videos. I understand that this consent will remain in effect until a written request to revoke consent is received by the club.

Understanding of Risks and Responsibilities
I understand that activities carry risks, including injury or death, whether caused by others or the environment.
I acknowledge Sonrise may not supervise all activities or enforce safety rules at all times.
I accept full responsibility for any injury, loss, or damage and release all Releasees from related claims.

Medical Authorization
I confirm that the medical information provided is complete and accurate.
I authorize Sonrise to obtain emergency medical care if necessary and agree to pay all associated costs.

Participant Behavior and Safety
I will instruct the participant(s) to check for any unsafe conditions and report them.
The participant(s) must follow all rules and directions set by Sonrise.
If the participant(s) fails to comply, they may be excluded, and I will be contacted to pick them up.

Legal Agreement
I agree to indemnify and hold harmless Sonrise and the Releasees from
any claims or legal fees resulting from participation.
If a claim is made despite this waiver, I agree to cover all resulting costs.
This agreement is governed by the laws of Alberta and remains valid even if part is found invalid.
I confirm I am the legal custodial parent(s)/guardian(s) of the participant(s) and are authorized to sign this form.

By adding my name to this form, I have read, understood, and voluntarily agree to this waiver.
