Use this form ONLY if you qualify as low income and have submitted your  income verification form THIS YEAR.  If you have not completed your income verification we won't be able to waive the fee.  Please pick up an income verification form in the office and submit it before submitting your request.  This verification MUST be done every year. If you DO NOT qualify as low income, you must pay $20 in the student store to reserve a test and you do not need to fill out this form.  If you DO qualify as low-income, fill out this form requesting your fee waiver.  You will be emailed confirming a test has been reserved for you and your fee is waived.  If you do not receive a confirmation email, you do not have a reserved test.  Please wait 2 school days for a confirmation.  This form must be filled out by September 10th at 3pm.  Please remember, tests are reserved on a first-come, first-served basis.  Those who fill out this form, or pay for their test first, are reserved tests.
