This document serves as an addendum to, and is a second communication of, regulations that includes information regarding Hayward Unified School District’s (HUSD) Acceptable Use of Technology Agreement (AUP) for the 2019-2020 school year as of April 2020.

During this time where Distance Learning is the mode for continuing the education of our HUSD students, teachers and students will be using digital services to conduct learning opportunities.  Many of these tools require that consent is collected from parents and guardians before using these tools with students, especially students under 13 years of age.  This addendum to the Acceptable Use of Technology Agreement (AUP) exists to provide families with the required consent forms to ensure HUSD is abiding by all Federal and State laws.  

By checking each box below you are affirming, or reaffirming, your consent for your HUSD student to use the Distance Learning services provided to your student.
