UAS Housing Item Storage/Return Request
You may use this form to request that:
1. Student Housing provide storage until you return to housing for Summer or Fall 2020.
2. Request that we send you immediate items that you require for coursework or wellness.
3. Request that we send you all items from your unit.
4. Request local pickup of your items.

Please fill out this form if you need personal items shipped to your permanent address. Student Housing will be prioritizing requests for individual items that are related to your wellness (i.e. medications) or coursework.

Requests to send all of your personal belongings will be a low priority at this time while we focus on the wellness and academic needs of our students who need items sent to them.

If you fall into BOTH categories (i.e. you have items you need now for coursework or wellness but also would like the rest of your items shipped at a later date), please fill out this form twice.

If you request that we store your items locally until you return, we will provide that storage until Fall 2020 move-in or when you return, whichever is sooner.

Please note by submitting this form you will be providing UAS Staff permission to enter your unit, collect your belongings, and mail them to you or store them for you. UAS does not take responsibility for the condition of your items upon arrival to your permanent address -- however, we will pack your items carefully and will do our best to ensure they arrive in good condition.
