Thank you for entrusting us with your child. Please fill out the following form and complete the hard or PDF copy of the credit card authorization. The $125 supply fee and first month's payment is due prior to your child's start date.

Please Initial if you accept the following: School Day/School Year-10 payments of $900 billed August through May. Schedule follows High Desert Montessori regular school calendar August 9th- June 3rd 7:30-5:30 daily on scheduled school days. *Summer breaks is not included *WCSD delayed starts or snow days will be observed as well. *

Please Initial: A Non-Refundable $125 Supply Fee is due with a signed contract in order to secure your child's spot. *

Please Initial: Tuition is charged month-to-month. We do not offer hourly, daily or weekly rates, Payment is due the 1st of each month. Any payment received after the 15th of the month will be considered late and will incur a $20 late fee. *

Please Initial: It is the responsibility of the parent to make all payments in a timely manner. HDMS reserves the right to exclude any student whose family has not made payments on a month-to-month basis. Exclusion can happen as early as 30 days past due, and remain in effect until tuition is paid in full. *

Please Initial: Re-enrollment is contingent upon the current account being in good standing, and available space. If you "forfeit" your space, due to payment delinquency, there are no guarantees your child's spot will be open for re-enrollment at any grade level. It is important that you make payments regularly and on time. *

Please Initial: Payments may be made in the form of cash, check, money order, or credit card. In order to avoid any problems with payment application, be sure to write your child's name on all payments. Please make checks payable to High Desert Montessori or HDMS. You will be charged a fee of $25 for any returned checks. If two NSF checks are received, payment will only be accepted in cash or money order form. For a cash payment, you may request a signed receipt at the time of the transaction. Without proof of payment, High Desert Montessori School will not be held responsible for any incurred loss. *

Please Initial: There will be a late pick up fee of $1 per MINUTE per child if the child is not picked up by 5:30 PM for "extended day" programs. The late pick up form will be filled out by the caregiver and you will receive a copy before leaving the premises. You will be billed for the $1 per minute. We are required by law to call Social Services at 6 PM if we have not heard from you. *

Please Initial: A 30- day notice is required for withdrawal. No refunds will be issued for any portion of payments received. If you choose to withdraw, you're responsible for the entire month's payment to cover the 30-day notice period. *

Please Initial: High Desert Montessori School reserves the right to use a collection agency to collect any unpaid tuition or childcare fees. *
