Please use this form to check out your own student-athlete from the meet. Please remember that it is your responsibility to check out with the head coach in person (both the student-athlete as well as the parent must be present) You may only take home your own student-athlete, no exceptions!

PLEASE READ:

I understand that District provided transportation was available and will not be necessary due to prior obligations. I understand that I take full responsibility for my child since they will be with me instead of with Coach Zarate and the rest of the team. I understand that I take full responsibility and release Coach Zarate, all Cy Woods Staff, and the Cypress Fairbanks I.S.D. from all liability in the event of any mishap or any unforeseen circumstance that may arise when my child is in my care. Upon digitally signing this document, I (parent’s Name) understand that I must have this waiver completed prior to checking out with Coach Zarate at the meet.
