OLC-Home Placement Change Form Q2 2020
Please complete this form ONLY if you are CHANGING your students' placement from school to home (OLC) or OLC (home) to school for Quarter 2 of the 2020-21 school year. If you are remaining in OLC or in school, there is no need to complete this form.

One form must be completed for each student. All forms must be submitted by Friday, November 6, 2020. If you opt for OLC, you will be receiving paperwork that must be signed and returned. Placement commitments are for 45 days, or one quarter, at a time.

I am requesting a change in placement for my student into the Olympian Learning Connection (OLC) remote learning from home option. By clicking I confirm, I understand that I will be sent paperwork to enroll my child into the OLC program, and expectations for participation in the program.
