In order to be considered to adopt a pet, you must:
 Be 18 years of age or older;
 If you are below 18 years, a parent or legal guardian should fill in the form;
 Have the knowledge and consent of your landlord if you currently rent;
 Have identification (of the same person who is filling out the form) showing your present address for verification purposes only;
 Be able and willing to provide a loving, safe and peaceful environment for the pet;
 Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care for the pet.

I understand that animals may be unpredictable and Paws and Hands United Inc. cannot guarantee nor warrant that an animal being adopted may not become aggressive. I understand that animals can be destructive and may cause damage to property. I willingly accept that risk and will not seek reimbursement or legal action against Paws and Hands United Inc. for any damage. This contract releases Paws and Hands United Inc. from any and all liabilities. IMPORTANT: THIS DOCUMENT RELEIVES PAWS AND HANDS UNITED INC., ITS BOARD OF DIRECTORS AND VOLUNTEERS OR ANYONE ACTING ON BEHALF OF PAWS ANDHANDS UNITED INC. FROM LIABILITY RESULTING FROM PERSONAL INJURY, WRONGFUL DEATH AND PROPERTY DAMAGE CAUSED BY THIS ANIMAL. I have read this document, understand it and sign voluntarily.
