As the parent or legal guardian of the above-named student, I hereby give my consent for my child to participate in the Running Club at Conn Elementary. I understand that participation in this club involves physical activities that carry inherent risks, including but not limited to, falls, collisions, and overexertion, which may result in injury.By signing this waiver, I acknowledge and agree to the following:
1. Voluntary Participation:
I understand that participation in the Running Club is voluntary and that my child is physically capable of engaging in such activities.
2. Assumption of Risk:
I acknowledge that participation in running activities involves risks of injury. I accept all risks associated with these activities, whether identified or unforeseen.
3. Release of Liability:
I agree to release and hold harmless Conn Elementary, its administrators, teachers, staff, volunteers, and all other affiliated entities from any and all claims, liabilities, or damages arising out of or in connection with my child’s participation in the Running Club, including any injuries that may occur.
4. Medical Treatment:
In the event of an emergency, I authorize school officials to secure medical treatment for my child. I understand that I will be responsible for any medical expenses incurred as a result of injury or illness during participation.
5. Behavior Expectations:
I acknowledge that my child is expected to follow all rules and guidelines set forth by the Running Club to ensure a safe and positive environment for all participants.
