Team and Sponsorship Registration - 2026

Use this form to register your team and/or purchase a sponsorship for LoveALatte Coffee's 2025 Mini-Golf Outing on Saturday July 11th at Adventure Commons (70 Ken Hayes Drive in Bourbonnais). Doors will open at 9:45, with our shot-gun start beginning at 10:15. Teams can be from 2-6 players, and the cost is $30 per golfer. Please note that small teams may be paired with another small team if the outing fills to capacity. Each player receives 18 holes of mini-golf, one-of-a-kind LoveALatte T-Shirt, lunch, and a free raffle ticket! This year's theme is "Christmas in July", and yes there will be prizes for the most festive and creative teams. We appreciate you and are looking forward to another fun outing! You may also use this form to donate a gift basket for our raffle!
