Parental Permission
Introduction
You and your child are being invited to be a part of the Berkeley Developmental Labs’ Research Participant Recruitment Database. This database is part of a collaboration between Mahesh Srinivasan’s Language and Cognitive Development Lab, Fei Xu’s Berkeley Early Learning Lab, Alison Gopnik’s Cognitive Developmental and Learning Lab, and the other developmental labs at UC Berkeley.

Procedures
If you decide to sign your child up for the Berkeley Developmental Labs’ Research Participant Recruitment Database, now online, you will be forwarded to a form which will ask you eligibility questions about your child/children. If eligible, you will be forwarded to further questions about your contact information and some questions such as birthdate, primary language, etc., about your child/children.

If ineligible, you will be thanked and exited from the online form and none of the information you have provided will be saved. If eligible, we will use this information to contact you in the future in order to connect you with studies your child is eligible to participate in. If you and your child agree to participate in one or more of these studies, you (and in some cases your child) will be asked to sign a parental permission and assent form specific to participating in that study.

Benefits
There are no anticipated direct benefits to you or your child for signing up for this recruitment database. By being a part of this database your child may have the opportunity to participate in interesting research studies. It is hoped that these research studies will help us further a scientific understanding of the cognitive developmental processes in children.

Risks/Discomforts
As with all research, there is a chance that confidentiality could be compromised; however, we are taking precautions to minimize this risk.

Confidentiality
Your/your child’s information will be maintained in the lab’s database as confidentially as possible. To minimize the risks to this information, only the researchers listed on this form and their research staff will have access to the database through a password-protected account on an encrypted and password-protected file. This information will be maintained in our database until your child reaches the age of maturity (18 years old in California) or if your family moves out of the San Francisco Area before that or if you/your child no longer want to be part of this database.
Rights
Participation in research is completely voluntary. You have the right to decline to allow your child to be a part of this database, or withdraw them at any point. Your child has the same rights to decline to participate or withdraw at any time

Consent
If you agree to have your child to participate in developmental research, please consult with your child/children (if age appropriate) about whether they would like to be part of our recruitment database and if they agree, please print a copy of this page to keep for future reference and continue to the lab sign up form.
