FEES and FIRST VISIT INFORMATION
  • Kelley PT is an out-of-network provider.  Invoices, including diagnosis and treatment codes, are provided after each session.  The invoice can be submitted to the individual's insurance company for eligible out-of-network benefits.
  • Kelley PT does not participate with Medicare.  Therefore, you will not be able to submit an invoice to Medicare for reimbursement.
  • Fees/Payment:  The fee for the 90-minute initial evaluation is $275.  Follow-up sessions are 60-75 minutes in length with a fee of $225.  Payment is required at the time services are rendered.  Kelley PT accepts payment via Zelle, personal check, cash, HSA, and credit card. Note: HSA and credit cards are subject to a 3% surcharge.
  • If it's been more than 4 months since your last appointment, the charge will be $250 to reflect the increase in time needed for reassessment and program update.
  • You MUST complete and sign the intake form prior to your initial appointment.  This is required by Maryland law.
  • Be sure to schedule at least 2 follow-up appointments, 2-3 weeks apart.  Doing so ensures that you are performing home techniques correctly to achieve their full impact and enables quicker progress for symptom resolution.  
  • Kelley PT is located at 4401 East-West Hwy., suite 303 (within 1440 Health Collective), Bethesda, MD.  
  • Parking is available in designated visitor spots or in spot 45 (reserved for Kelley PT patients), located behind the building.  Additionally, public parking is accessible in the garage situated across the street, one block West, towards Wisconsin Ave.
  • Wear comfortable, loose-fitting clothing such as athletic wear.  Wear or bring the shoes that you workout in.
  • Bring your prescription eye glasses and mouth splint, if you have these. 
  • Cancellation/No show:  Kelley PT places a tremendous value on the time to work thoroughly and consistently with each patient. For this reason, two business day's notice is required prior to your appointment if cancellation is necessary.  Any missed appointments or cancellations within two business day's notice will result in a fee of $100.
