Welcome to Wellness Minneapolis's direct lab ordering form. It's our mission to empower people to take charge of their health so we are making it easier for you to access bloodwork. 

*This form is NOT for existing patients. If you are an existing patient please send a message to your provider in Charm about having labs ordered.*

After this form is submitted you will receive an electronic invoice from Wellness Minneapolis. Once the invoice is paid it will take up to 3 days to process your order. 

1. You will receive an email from CharmPHR about setting up your secure account to receive your lab results and sign consent forms. 
2. Your Charm Portal is where you will find the order form and collection instructions.

Refunds/Changing the Order
Refunds: refunds are available for 50% of the cost up to 6 months after the labs are ordered. After 6 months no refunds are available. If your blood has been drawn we are unable to refund any amount. 

Changing the order: If you would like to change the bundle you picked please email the office at info@wellnessmpls.com. If the labs have already been ordered there will be an additional $25 fee to make those changes. 

Substitutions: We do not allow for additions or subtractions from the bundles at this time.
