HYPU's Holiday CARES Act for Youth Application

Reminder that if you have already filled out this form, to complete the application process, please submit the following documents to covidprograms@hypu.org. You can send as a scanned filed or phone image. Incomplete applications will not be considered.

Document Verifications:
* Must provide proof of address (i.e. ID, DL, correspondence, utility bills, etc.)
* Must provide following identification documentation for each child (i.e. screenshot of online document, report cards, birth certificates, etc.)
* Must provide the last 30 day paystubs and/or income documentation

Upon Selection:
An email will be sent out by December 4th to confirm the selection of your application along with detailed information regarding the drive-thru and your pick up time/appointment.

Picking up Items:
Only individuals listed in the Parent/Guardian section or the Driver section will be allowed to pick up for the family. A valid Driver's License or ID is required at check-in for verification. Unlisted names will not be allowed to pick up for the family.
