Religious Education Registration 2020-2021
Thank you for considering St. Christopher Parish for the religious education of your child(ren). If you have not already done so, please read through the information on our website (https://www.saintchristopherparish.com/2020/08/18/2020-2021-religious-education/) or included in your enrollment letter regarding adjustments to our curriculum and format for the 2020-2021 school year to account for the ongoing coronavirus pandemic. Because community is so vital to growing in our faith, and in accord with Diocesan guidelines, we are pleased to offer in-person formation for all levels that will follow the appropriate safety protocols from the Dane County Public Health Department. Those who have concerns related to COVID-19 will also have the option of registering for virtual faith formation this year. This form can be used to register for all levels of religious education.

Please direct any questions to Amy Harker (aharker@saintchristopherparish.com) or Steve Davies (sdavies@saintchristopherparish.com). Registration via this form is due by Tuesday, September 1.
COVID Information
Please review our COVID updates page (https://www.saintchristopherparish.com/coronavirus/) for a full description of the Saint Christopher Parish safety protocols and procedures. In addition to measures put in place by the parish, all families are expected to monitor for symptoms and comply with policies set forth by the Public Health Department. Specifically, parents must agree to check the temperature of children daily before they attend any faith formation event. If children have any symptoms of sickness (temperature, persistent cough, runny nose, vomiting or diarrhea), they are not permitted to attend the event on that day. Children should be symptom free without medicine for 24 hours before returning to the parish.
State your name below to indicate you have read and agree to comply with the parish COVID policies. *
