Note that fee waiver process is separate from the application process and receipt of a fee waiver is not a guarantee of admission. This form must be submitted by Nov 6, 2026 to receive full consideration for the fee waiver. We anticipate that decisions will be made by Dec 1, 2026 (ahead of application deadline of Dec 15). 

NOTE:  To receive full consideration, you must have uploaded/submitted all the application materials to the graduate school portal, so that the status is "Awaiting Payment".
