Val Verde Academy Absence Verification
Please use this form to report your student's absence. STUDENTS CANNOT EXCUSE THEIR OWN ABSENCES. THIS FORM MUST BE FILLED OUT BY A PARENT/GUARDIAN. 

Students are allotted 10 EXCUSED absences per school year. Please provide a medical note for any absence related to an appointment for: doctor, dental, eye, therapy, labs etc. Medical notes are exempt from the allotted 10 EXCUSED absences per school year and are protected for such appointments on record.  Complete all required fields below to E-Verify your child's absence(s).
