Along with this form, please print out and hand in the 2020 USAU membership form and the 2020 USAU medical authorization form. They can be found here or on the registration page of the Cathedral Ultimate website.

The completed USAU forms and payment for registration, uniforms, and discs can be turned into the Cathedral Athletics Office or mailed to the address below. Please make checks payable to Cathedral Ultimate and place with the forms in an envelope marked Cathedral Ultimate Registration.

New for Middle School - Team Owned T-shirts
Middle School players will wear black shorts, along with a blue and a white t-shirt that will be provided. Both t-shirts will need to be returned to the Cathedral Ultimate Frisbee Club at the end of the 2020 season. (Middle School players are also welcome to purchase the new uniforms as a pack for $65).  Please indicate size selections for each piece below.  
High School Player Uniforms (New Design)
All High School Players (New and Returning) will need to purchase new uniforms as we have a new design and colors.  The uniforms will be sold in packs including one blue shirt, one white shirt, and one pair of blue shorts at a cost of $65.00.  Please indicate your size selections for each piece below.
