Medical/Legal Release

Participation in any sport or sport-related activity could be a dangerous activity involving risks of injury,
including significant impairment of long-term health and well-being, and serious injury to virtually any part of
the mind and body. Each player and his/her parent or legal guardian therefore must confirm that the player is in good health and physically able to train and play in the RNY FC Youth (Empire United Soccer Academy) games and events, and further has no medical conditions or physical limitations that would prevent him/her from fully participating in the activities of RNY FC Youth (Empire United Soccer Academy). Each player and his/her parent or legal guardian also must immediately notify both RNY FC Youth (Empire United Soccer Academy) and the respective coaching staff of any conditions developing during the season that may prevent the player from continuing to participate fully in the activities of RNY FC Youth (Empire United Soccer Academy). Each player and his/her parent or legal guardian must also waive any and all claims against RNY FC Youth (Empire United Soccer Academy), its directors, officers, employees, coaching staff and volunteers by reason of any physical, mental or emotional injuries or otherwise alleged to have been caused by participation in any activities of the Club.

COMMUNICABLE DISEASE AND RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT

In consideration of being allowed to participate in any way in the program, related events and activities, I the
undersigned, acknowledge, appreciate, and agree that:

I am aware there are risks to me of injury from participation in team training activities and/or of exposure to
directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all
communicable disease.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE RNY FC Youth (Empire United Soccer Academy), its officers, officials, agents and/or employees, other participants, sponsors, advertisers, players and parents and, if applicable, owners and lessors of premises used to conduct the event (RELEASEES), from any and all claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I may suffer, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS
TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND
VOLUNTARILY WITHOUT ANY INDUCEMENT. FOR PARENTS/GUARDIANS OF PARTICIPANT OF MINOR AGE
(UNDER AGE 18 AT TIME OF REGISTRATION) This is to certify that I, as parent/guardian with legal responsibility for this participant, consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release the Releasees from any and all liability incidents to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law. AGREEMENT By virtue of registering my child or the child for whom I am a legal Guardian for, I confirm having read and understand the Commitment Policy, Medical Release and Hold Harmless Agreement and I confirm that I or any players for whom I am a parent or legal guardian am/are in good health and physically able to train and play in RNY FC Youth (Empire United Soccer Academy) games and tournaments with no medical conditions or physical limitations preventing full participation. I also agree on behalf of myself, any players for whom I am a parent or legal guardian, and my and their heirs and assigns:

1. To use best efforts to attend and participate in all activities of RNY FC Youth (Empire United Soccer Academy) as are requested of me;

2. If a parent or legal guardian, to assure that those players for whom I am responsible will attend and
participate in all activities of RNY FC Youth (Empire United Soccer Academy) as are requested of them;

3. To timely pay to RNY FC Youth (Empire United Soccer Academy) the full tuition and expenses as are required of me;

4. To immediately notify both RNY FC Youth (Empire United Soccer Academy) and the respective coaching staff of any conditions developing during the season that may prevent continuing participation fully in the activities of RNY FC Youth (Empire United Soccer Academy).

5. To hold RNY FC Youth (Empire United Soccer Academy), its directors, officers, employees, coaching staff and volunteers harmless from any and all liability, claims and demands of every kind that may arise by or in
connection with participation in any activities of RNY FC Youth (Empire United Soccer Academy).
