PTA Member & Student Contact information
Dear Families,

In an effort to streamline our member and student contact information, we are asking all families to complete the attached form and submit it to the PTA Mailbox at either PVES or PVMS. We are making an Elementary school wide directory and this information will also be kept for all future PVES classroom directories (*if you consent to having your contact information being shared for this purpose). Once you fill this out, you will not have to complete another one. If there are changes to your child’s name, phone number, address, or allergy information*, please contact us as soon as possible to update this information.

(*The PV PTA will NOT share this information with any other entity)

Please note, this is NOT your annual membership form and payment. This is simply to update information. To become or remain a PTA member, you must complete the PV PTA Membership form and submit with payment or purchase online.
