Please only use this Form to submit a Health Report or cause of death if you are a UK BMH breeder or owner. This form should not take longer than 10 minutes to complete. There are 24 sections in total depending on your answers.

You can use this Form to enter a Health Report for ONE BMH  (born any time from 2000).  Please submit a new Form for each additional BMH.

By using this Form you agree to the BMHS of GB using the data you supply to inform their Health Plans and to provide reports on the health status of UK BMHs.

Your personal details and those of your BMH(s) will not be shared publicly and will only be used, in confidence, by the BMHS of GB and Health Co-ordinators.  

Remember to let your dog's breeder know about any serious health conditions.
