Please use this form to notify the Department of Recreation whether any injuries occurred during a club or intramural sport event. This form should be completed after each competition, regardless of whether any injuries occurred. If a member is injured during practice, this form should be used to report that injury as well. It is not necessary to complete the form after every practice if there were no injuries.
